I experienced the same issue - the problem appears to lie with the "content-disposition" field in the message headers. Apple Mail always seems to set this to "inline", regardless of what settings you change ("View as Icon", "Windows-friendly", "Attachments at end" etc.) and, for some reason, Outlook users are unable to save these attached images files.
Finally, I discovered http://lokiware.info/Attachment-Tamer which is an add-on for Apple Mail and which can force the content-disposition field to "attachment" so the receiving Outlook client is able to save the attachments as normal. I have literally just been testing this myself and it seems to have done the trick.

This does not change how the message is constructed. Attachment Tamer is the only solution.

One thing you can do is turn it into a zip file (control-click, compress) and attach that.

I agree with etresoft's response. I use Attachment Tamer and it's the only way to customize how images and documents are handled in Mail. Well worth the $15.
It's easy. It has to do with your email signature (something to do with HTML...). If you manually type your signature instead of copying and pasting it, the pictures will be sent as jpeg attachments instead of being embedded in the email. I found this on another forum and I couldn't be happier!
I have tried every tip and trick I can find (dragging and dropping, going into Applications>Utilities>Terminal and typing "defaults write com.apple.mail DisableInlineAttachmentViewing -bool yes", checking [√] Send windows-friendly attachments when clicking the paper clip icon to add an attachment) and this simple fix worked!
I removed my signature completely from the email message by going to Mail > Preferences > Signature and deleting everything I had in there. I don't think it is necessary to remove it via Preferences and a simple "delete" will do but I don't really need my signature on my personal email account anyways and don't mind losing it.
I decided to go back and remove the DisableInline command to test if that would have an impact. I restarted mail and sent a test email to my work computer which is a PC utilizing Outlook. The attachment became embedded once again. I added the DisableInline code back in, sent a test email, and voila! Attachment returned!
So, it seems the cause of this issue is multifaceted like most problems in life. If you have tried all of the options listed and are stll having issues it may just be the automated signature is the culprit. Good luck! I will be reposting this on some other forums I used to get to this point. 🙂
Sorry to burst your bubble, but this is not a fix per se. It is simply taking advantage of yet another PC e-mail bug. An image followed by additional text actually gets treated as an image attachment followed by a text attachment. This particular sequence will cause some versions of Outlook to choke on displaying the attachments (all of which are still inline, by the way) and they will be displayed as attachments with icons. There are plenty of threads here demanding that Apple fix this Outlook bug too.
