The easiest and least expensive way is to use the abilities of built in programming on the Mac.

You can easily make an Automator App to perform the task. Understand that the output is not always pretty, as certain characters, lines and images are converted to gibberish. But's an easy task to open the text file and do a "search and replace" to get rid of extraneous unwanted characters.

Here's a tutorial as to how to make an Automator App for what you want to do. It leaves a bit out, as it presumes you already know how to use Automator a bit. When you open the application, select "application". In step # 2, you have to find the script in the left panel, under PDF files, and drag it over to the right side. Don't use the RTF option, instead use "text". Or you can experiment both ways by making two different Automator apps.

Adobe also has a way to fill in blanks. I can't remember exactly where I found the option, but at one point in time I did use Adobe Reader to get a form and was able to fill it in and print it out right through Adobe Reader.
