Where's Name field in Adressbook Now?

I can no longer find the field that I think was called "Name" and it was a drop down menu for

Father

Mother

Parent

Brother

Sister

Child

Friend

Spouse

Partner

Assistant

Manager

Other

....

Custom

On the records that I had this field invoked, it's there,

On new records though, in Edit I've clicked all the green + add buttons and

can't find it.

Where did it go, how do you add this field now?
