I have an iPhone 4 16GB that has recently lost all connections: unknown IMEI, unknown ICCID, no wifi etc. I know that it is probably a hardware issue and I could probably take it to my nearest Apple Store for a replacement because it is still under warranty until November. My front screen is cracked though. Would I still be able to get a free replacement since the hardware issue is covered under warranty?

Cracked screen generally voids the Warranty or AppleCare Coverage. Taking it to Apple never hurts however, Genius will be glad to hear your story.
Anyway, try this, tap Settings App>General>Reset near bottom of list>Reset All Nework Settings.
I've tried resetting network settings, all settings, restore, restore from DFU, used Windows PC and MacBook Pro... I've tried everything. I've heard of each issue ( unknown imei and cracked screen) separately but haven't seen a situation like mine before.
Has anyone else been in the same situation as me and successfully received a replacement iPhone under warranty?
With a cracked screen, why would you expect to receive a replacement under warranty? You've clearly voided the warranty.
Expect to pay the 199.00 out of warranty exchange. That is the norm. If by chance a Genius waives the fee (it has been known to happen), consider it a gift. It should NOT be an expectation.
That leads into my next question. I live in Canada and the phone is locked to Telus. If they were to give me a replacement phone, would it be locked to Telus or could it be an unlocked iPhone?
You get a refurbished replacement the same as your original one. If your original was locked to Telus, so will be your replacement.
