Try this exactly:

1. Turn LED Alert off.

2. Press Home.

3. Press Power+Home buttons at same time and keep both pressed until the apple logo appears during startup. Letgo.

4. Go to Settings/General/Accessibility/

5. Find the LED Alert option that shows its self switched off.

6. Press and hold on the LED Alert's Off Switch.

7. DRAG THE SWITCH to the ON position.

8. If your custom vibrate setting isn't turned off then:

9. Like #7 DRAG the custom vibe switch to the OFF position.

10. Press HOME BUTTON.

Now have someone text you or set a one minute alarm to check it.

Two or more of the 10 steps fix the problem. I don't have time to narrow down exactly which ones. I did at one point have the LED Flash Alert and Custom Vibe on and working together, I'm almost positive. So if you get it working and like the custom vibe feature then try turning the vibe back on via dragging (not pressing) the switch.

GL and let me know if it works or not.

Read my above post / reply to your question.

You could try that second and try this first:

1. Hard Reset iPhone (holding power+home until apple logo)

2. Settings/General/Accessibility

3. Turn LED Alert OFF by holding the switch and sliding your finger to the off.

4. Turn Custom Vibe OFF by holding the switch and sliding your finger to the off.

5. Turn both LED Alert and Custom Vibe back on by sliding the switches.

Just never press those switches. Always slide them to your desired setting.

Go ahead and test it out by setting a 1 Minute alarm.

For some strange reason, mine seems to have started working - at 5AM. I did try the slide trick, but I am very skeptical of that "fix" - if the switch says ON, it should be registered that way no matter how it got activated (sliding or tapping).

Anyway, I did notice the flash, but I can't be sure it's working consistently.

I can confirm that it did flash only while locked/asleep, and this is noted in the User Manual. It makes sense - if you're using the phone, why flash it, especially when you could have it pointed at someone's eyes?
