I do not have a Contacts or Address Book icon on my new iPhone 4s. I either trashed it or it does not come with one but I would like one just the same. How can I restore/install a contacts or address book icon?

I just purchased a new iPhone 4S and couldn't find the Contacts icon. It was in the Utilities folder - check there. Hope that helps
The contacts icon can't be deleted. You may have either inadvertently moved it to another screen or it's in a folder, try looking in a folder called utilities as this seems to be a common folder for it to be in. The other options is to go to spotlight and do a search for contacts. It won't tell you where it is but at least you will know its on your iPhone somewhere.
This isn't crappy Windows, you can't "trash" a pre-installed app...
Settings > General > Reset > Reset Home Screen Layout
