Click on System Preferences, Sound, select "output" and if you have headphones plugged into the jack, you should see headphones/headphone port. Look at the bottom of the window and you may see the mute box checked. This happens on my computer and I ended up using my brand new macbook pro for a year without headphones before I figured this out. Nobody at the apple store explained this to me either.

OS X Mountain Lion: Reset your computer’s PRAM
A small amount of your computer’s memory, called “parameter random-access memory” or PRAM, stores certain settings in a location that OS X can access quickly. The particular settings that are stored depend on your type of Mac and the types of devices connected to it. The settings include your designated startup disk, display resolution, speaker volume, and other information.
Note: To print these instructions, open the Help Viewer’s Action pop-up menu (looks like a gear) and choose Print.
Resetting PRAM may change some system settings and preferences. Use System Preferences to restore your settings.
I have only been able to use apple headphones for a couple years now. Thank you for the solution. I can use normal speakers or headphones now.
Oh Man, I was about to replace the sound board or something in my MBP. Buy turning up the volume did the trick.

Didn't even think to check preferences, mine was on 'mute'! Everything working fine now.
I can't even to plug all of my earphone jack. There is a red led that do not let me insert completely the plug.
The same, headphone jack just does not work....

If you have software installed in your Mac to stream audio, sometimes they change your output sound settings.
What I did was to open System Preferences > Sound, then select "Output" tab, as someone recommended above, and check what's selected in the box "Select a device for sound output", if there is another device selected, just select Headphones.
None of the other solutions worked for me, this one worked, but my case was that "Headphones" were not selected as "Output".
I know this sounds crazy, but if you have your heaphones plugged in and turn the volume down, the next time you try listening by plugging back in there is no volume. So you have t go to preferences and turn the output volume for headphones up. They just don't want to fix it.
I had this problem: the audio was louder in the left ear than the right. It seemed like my headphones were broken. It happened with two different pairs, so I then thought my audio jack was jacked up.
It turns out, the balance in System Prefs - Sound - Output - Headphones had somehow been changed. The headphone option only shows up in prefs while headphones are actually plugged in. Problem solved. Thanks, everyone!

This worked for me too! On my laptop speakers it was full volume then when i plug the external speakers in it was on mute! A good safety device so you don't blast your ears off I suppose if using headphones! Thanks for the tip!

Another solution for headphone problems:
My Powerbook Pro has two jacks (input and output) and so plugged my headphones into both using the Y adaptor in the package. After selecting "headphones" in system preferences--> sound --> output, I had sound in the headphones but the mic didn't work. "External microphone" did not show as an option in the input window.
I removed the Y-adaptor and plugged the single wire into the headphone jack. Now it works! "External microphone" shows up and functions correctly.
A clue was found in the system profiler where selecting Audio --> Intel High definition Audio shows: "Headphone connection = combination output". The combination audio plug has three bands; separate input/output plugs only have two bands.
