What kind of Safari enhancer do you have on there?
An extensionm plug-in or app?
Disable Resume > How To Disable Lion's 'Resume' Feature - MacRumors.com
Then restart your Mac, launch Safari. Try YouTube.
If that doesn't help, quit Safari if it's open.
Go to ~/Library/Caches/com.apple.Safari
Move the Cache.db file from the com.apple.Safari folder to the Trash.
Launch Safari, try YouTube.
~ (Tilde) character represents the Home folder.
For Lion: To find the Home folder in OS X Lion, open the Finder, hold the Option key, and choose Go > Library

I don't remember doing this, but I had a "clean youtube" extension installed.
Now that's gone and everything is back to normal,

Nope, and that was part of the problem, because there was nothing obvious to remind me that I had that extension installed.
Those icons are from other extensions that have not caused me any trouble (yet!).
For the record, from left to right: - fades the page to dark while watching videos - closes all tabs but the one you are on - add a tab - resize the window

What extension are they from? They're new to me . . .
Interestingly enough I cannot find the tab ones here https://extensions.apple.com/
The one that switches the lights off while watching videos is towards the bottom of the page in the "Entertainment" category; this is the developer http://www.stefanvd.net/
If you have problems in the future, I would immediately suspect those extensions. Maybe turn off all extensions and try it again afterwards. But hopefully you won't have any more problems.
