I'm considering purchasing a MacBook for all of my computing needs and getting rid of my clunky desktop. I have a wonderful 21 inch Apple monitor that I'd like to continue using. Is there any device available to easily connect, say a 13" MacBook, to an external monitor?

Use an adaptor for that type of display and the computer's monitor's port. If the computer has a Thunderbolt port, use a Mini DisplayPort adapter.

Yes, there are adaptors to connect the Mini-DisplayPort to the style of cable required for your external display. You can find them on the Apple Store.
