You could call AppleCare and carefully explain the situation to them. As way of explanation, all I can say is that this happened to a member of our family and I think it turned out that a third party was making inappropriate use of an AppleCare registration. That may not be the case here – it may just be a mistake (a screw-up with serial numbers, perhaps) – but I think it's worthwhile checking.

Here was the url...

Unsure if clicking could cause issues. I called Apple but they are out of the office. Their 1-800 autobot is questionable at best. I'll give 'er another shot tomorrow. I have two accounts, an American one and a Canadian one, this is from the Canadian one. I ruled out iTunes. I did get an odd notice about my iTunes on the American account but it told me to delete the message if everything was good on my end.

Hi LieBerries!

I know it's been a while when you ask why you got this survey on this forum, but I think I have a simple explanation of what happened for you:

Grosso modo: you probably gave away or sell an Apple device that was registred with your basic informations to somebody else, then that person called the phone tech support about that same device. The advisor who took the call tried his best to please the customer and resolve the issue as fast as possible with the best quality in call handling (exemple: wifi is not working: reset: wow! it's working thank you!) Sometimes those calls are so quick that the advisor don't even have the chance to change the informations related to the device. Then Apple send random email surveys to monitor counsumer satifaction.

Those informations that can be viewed by advisors are very limited (name, address, phone no. and of course AppleID) and any other infos (like billing infos) are not acessible for a simple phone advisor.

If in any way you feel that the informations you gave to Apple needs a review, I invite you to call 1-800-My-Apple and describe your issue.

I hope you got the awnsers you needed!

This is definitely a HACK ATTEMPT... I consider myself pretty savvy, and fell for this one - darn it as I clicked on the Apple Care link, on my iPhone and completed to fake survey! So, I'm up at 4am resetting my Apple password and hoping that there is minimal damage caused by malware.

I received pop-up relating to "inability to verify server identity of "caldav.calendar.yahoo.com" which according to google is sign of malware." Apparently, the intent is to grab credit card info stored on iOS device(s) and it's my hope the fall-out if minimal, since I caught it quickly.

Sigh, it looks like this hack has been going on for years, and I'm disappointed that I fell for this one. Hopefully, I'm a bit smarter now (and can go back to sleep)!
