Same way you delete any other file—locate and select the file, move it to the Trash.
Go to the Finder to do this, then:
yourname > Library > Application Support > iWork > Pages > Templates > My Templates
If you've moved to Lion, your Library folder is hidden. To see it, press and hold option, go to the Go menu and choose Library, then follow the rest of the path above.

There's another way to remove the Custom Template that's more useful and what one would expect from Apple:
on opening Pages, go to My Templates, and two-finger-click (or cntl-click / right-click) the template needed to be deleted. The two options will be to rename or to delete such.

Historically, there's no reason for this to be "what one would expect from Apple." Since the first Mac, file deletion has been a Finder operation, accomplished originally by selecting the file(s) then dragging the file icon(s) to the Trash.
Assuming that "go to My Templates" in your post means "go to the My Templates section of the Template Chooser," your described method was apparently introduced in Pages 5 and/or Mavericks.
Using Pages 4.3, OS X v10.8.5, I tried a two finger tap and a control-click on one of my custom templates with no effect other than the template being selected. No menu appeared, no other action occurred.

This has been a (long overdue) improvement in Pages 5.
Pages 5 actually does have a few improvements. If only there weren't so many downgrades.

Barry, YouCAN'T delete templates like you discribed out of your Ipages, because they are permanent.
What you do is open the template window, rightclick ( 2 fingers on touch pad), select the template you want to remove and press delete.
That works for me.

That works for you because you are using Pages 5.
Apple has made a 'few' revisions to Pages (no "i") with the current version, including the method of deleting custom templates.
The method you describe (right click on icon in the Template window, press delete) works with Pages 5, but produces no result with earlier versions.
The method I described works with Pages '09 v4.3 and earlier. It was correct at the time of posting (December, 2011), and for those versions of Pages (and the other iWork applications) remains correct today.

PS: If the templates "CAN'T be deleted... because they are permanent," how is it that the method you describe results in their deletion?
