my phone app is gone what can i do to fix it?

my iphone is having a problem with the phone app. i just set my iphone down for about a hour and come back and the phone app was gone. What can i do to fix this

iPhone 4, iOS 5.0.1

Settings/General/Reset - Reset Home Screen Layout

Turn on the phone
Press the HOME button to go to the first Home screen
Find the SETTINGS app and tap on it
Find the line that says General and tap on the word "General"
Find the line that says Reset and tap on the word "Reset"
Scroll down to the line that says Reset Home Screen Layout and tap on it
