How do I create a text box in Pages '09?

Such basics are covered in the Pages User Guide, downloadable from the Help menu. It would be a great idea for you to read the guide because there are bound to be future occasions when you will benefit from having done that.

For now, Insert > Text Box.

It is an icon on the Toolbar. The box with "T" in it. Usually lablled underneath as well.

This is basic stuff, take the time to look through your menus, the Toolbar and the Inspectors.
