I also didn't have a PH on my new iPhone 4s. A quick chat with the provider resolved that, it's a bug with the IOS software, sometimes it's there other times it goes. Moving ffrom IOS5 to 5.1 also removed the option.

Settings - Mobile Data Network - Reset Settings

Forces a restart and then PH is displayed.

You may need to reenter any WIFI passwords.

I'd faced the same problem as you.

I just solved it at the moment.

These're my solution :

- Setting > General > Reset > Reset Network Settings

- Setting > Carrier > turn off Automatic > Tap ur Current Carrier

- Setting > General > Network > Cellular Data Network > on all APN put ur carrier (Call to ask ur Carrier company)

And then you'll see ur Personal Hotspot in Setting > General > Network > Personal Hotspot

Set ur Wifi-Password

And It's done ^_^

Wish u good luck
