Feb 4, 2012 2:27 PM in response to jweisenby MajWoops,I had the same problem, and got into the tools using the fix linked above.
After, I updated the Server Admin Tools to version 10.7.3, and after a reboot it worked as expected. I didn't do enough testing beforehand to say this is THE solution, but it might work for you as well.

When i follow the additional steps using terminal, the second command brings back the same outcome - 'David is not in the sudoers file. this incident will be reported'.
I've been having trouble with login on for a week but didn't realise this was the problem. I've reset passwords, reinstalled OSX (twice) and now, bizarrely, i can log onto the mac mini as the administrator, but once logged in, the 'administrator' password doesnt work for anything. I can run Software Update because it comes up with a message:
"Software Update wants to make changes. Type an administrator name and password to allow this".
I then put in my password but it shakes and tells me its wrong.
Can anyone advise any other solution? I'm kind of stuck - because the system doesn't recognise my admin password for updates, i cant update anything. And also because my itunes library was set up by a newwer version (I reinstalled OSX believing it would fix it reverting it back to a previous version) i can't listen to any music.

Feb 6, 2012 9:57 AM in response to da_vide78by MajWoops,I experienced the "this incident will be reported" line, but realized I was logged in as a regular user. Once I logged in as an Admin and ran those commands, it worked.
Although with my (limited) understanding of sudo, I don't think you have to be logged in as a super user/admin (that's why the sudo command exists).
When you log in as an admin, what's the result of running the second command from the KB?

I spoke to Apple care tonight who said best thing to do right now is wipe and reinstall. Luckily i have time machine back up..

Same here. I had same problem with upgrading from a Lion Server install that had gone from 10.7 to 10.7.2 and was working fine. But the upgrade to 10.7.3 broke my admin login and sure enough this apple support item worked when I used the CLI commands. Now my 10.7.3 server is running fine.

None of the solutions suggested above worked with me. I have tried the command line thing but it keeps telling me that my user name is not in the sudoers file. Although im using my admin name and password.

Apr 2, 2012 1:52 PM in response to malgharaby da_vide78,I had a similar problem but mine was to do with permissions. In the end, i spoke to the AppleCare guys who suggested to do a full reinstall after reformatting the HD. Sudo list issues may be fixable, but speak to a professional first. I was surprised how helpful the AppleCare guys were even though i don't pay for the extended service.

May 12, 2012 12:34 PM in response to jweisenby John Benninghoff,I had a similar problem, couldn't log in locally. This helped: http://support.apple.com/kb/TS3960, but I had to unload servermgrd first:
sudo launchctl unload -w /System/Library/LaunchDaemons/com.apple.servermgrd.plist
sudo launchctl load -w /System/Library/LaunchDaemons/com.apple.servermgrd.plist
I suspect a reboot would have also fixed it.
