Is there I way to check my macbook's fan health or an app for it?

My macbook is getting older, it uses Lion OS though but is there a way to check my fan's life or something?

No. There is no such thing. Either the fans work or they don't work. You can install various hardware monitors that provide information on fan speed and hardware temps, but that's about it. See

If the fan start failling how will I notice? Will the Mac get damage or does it have some kind of self protection?

Well, it may start making noises, the computer may start overheating, or you may no longer hear it running.

The self-protection is that the CPU will shutdown the computer if it's internal temperature hits 125ºC.
