Can anyone help me?

Is there an app that we can use as a docket book? What we are trying to do is have our service eng attend site to complete a job. We want them to fill in a report sheet telling us what they did on site, when they arrived, when they finished and what products where used. They then hand the iPad or iPhone to the customer for a signature, a copy of the docket is then sent to our office PC and also the customer. Is there a system out there for this? It would be great if there was also a Mac version to sync with as well.
