#1 - it can vary a bit from 7-9 days to 2-3 weeks. I've had books come in in about 9 days. Others have reported nearly 3 weeks. All of mine were printed on the west coast where I'm located. Don't know if that is the major factor.
#2 - The paper is of the glossy type, similar to magazine paper.
#3 - To improve the image quality a bit you can use
Pref Setter to change the DPI settings in the iPhoto preference file as follows:

#4 - all of my books were gifts, long gone.
#5 - no what you see is what you get. The Folio theme has a page layout where you can one or two pictures at the right or left and the rest of the page open for text and/or signing. The Contemporary theme has a page layout where the photo is in the center of the page and there's plenty of room surrounding the photo for signatures/comments. You can check out the other themes and page layouts to see if there's something you think will work.
You might look into a pen that will write on glossy surfaces and stay indelible and not smear after drying. One I use for marking glossy prints is:
PILOT, SCA-UF, ultra fine point, no xylene permanent type.
It'll even write and stay on glass. You can get them at office supply stores.
