You can login to your acoount in iTunes and find that purchase. Click on report a problem and that will enable you to click on that individual app. Fill in the requested info and you will have a refund in a few days.

I haven't been able to get to "report a problem" with apps for about a week, either in the app store in iTunes on the iPhone or my computer.
Is it just me or is it system-wide?

I was able to login and view my past purchases on my MacBook just now. What kind of computer are you using and how are you connecting? If you are having trouble when using WiFi on the phone and computer, something is wrong with your WiFi.
I can view purchases, make purchases, browse and search for content and apps and all the rest on both my desktop PC with iTunes (Vista SP2) and the App Store on iPhone 4 (ios5) but can't get to "report a problem" on either.
Connection type on the iphone doesn't change anything.
Maybe the App Store doesn't want my input anymore?
some one steel my account to purchase some items from onother computer .
i just receive 3 bills . with mor than 16 items purchesd by him .

What do you mean 'hit Test deductions will be returned when' ?
If you buy an app then you've bought it, you will only potentially get a refund if the app doesn't work (or doesn't work as described) and you contact iTunes Support and they agree to refund you - in which case the refund should happen within a few days of them telling you that they will do so.

If you are referring to temporary store holding charges then your card issuer should remove them from your card : iTunes Store & Mac App Store: About payment card authorization holds.
Otherwise refunds are only potentially given if you contact iTunes Support and they agree to refund you
