FYI - solution is to fill in the APN information in the "Internet tethering" section of network settings. This seems to be a new section for IOS 5.1. Once this information is set, the personal hotspot option appears again.

when i first upgradexd to 5.1 (last friday) my personal hotspot was also gone - i tried all the suggestions i could find on these forums but nothing worked - mysteriosly it just cam back today - and i havent done anything like a sync or reset - im wondering if it was something the phone company did (in my case ATT)
