No. After it Is completely downloaded you do not Internet in order to watch the movie.
In the interest of full disclosure, I can tell you that I have gotten inconsistent results from this. There have definitely been times when I have a fully downloaded iTunes rental and have gotten a message saying an Internet connection is required for authentication when trying to view it offline. Last week, however, I did the same and had no problem watching one without Internet. Perhaps they changed the policy recently, or else there is a bug somewhere in the system.
Do note, however, that the following Apple support article states that an Internet connection is needed to authorize playback of movie rentals on the iPad:
It looks like the requirement for Internet connection only applies to HD (High Def) movies but not to SD (Standard Def) movies? Can Apple confirm?
