having trouble with iCloud

i am having trouble with duplicate entries on my calendar and contacts and reminders. I have a Mac desktop and and iphone 4 that i'm trying to synch. I use MS Outlook. how do i eliminate all the duplications?

iPhone 4, Mac OS X (10.7.3)
