the 16 px rule does not work anymore

I have been putting out QT videos for a while now and just recently I found that I needed to add MORE THAN 16 px for the movie and controls to appear.

Has anyone else noticed as well?

If so , what is the new rule?
btw: I use dream 2004

16 pixels works just fine for QT files that have a video track. The controller displays just fine.
But I've noticed audio only files need a minimum of 18 pixels for height (16 pixels is still the minimum for width) before they display properly in Safari (and some other Mac browsers).
I now use 18X16 size for audio files to display the on/off portion of the controller.
