Does your carrier support tethering? Your cell phone carrier needs to activate the personal hotspot on the iPhone before it shows up. Some carriers do not support it, some do for a fee, and some turn it on automatically including it with the iPhone package.
Got to settings>general>network and in here you should find personal hotspot. If it isn't in here, you will need to speak to your carrier whether you can have your account provisioned for personal hotspot.

I feel a bit lost here - my IPhone does not even have the 'Network' option under 'General' - so I guess the Personal Hotspot is not for me...
This is of course annoying, in particular because it seems to involve bottom-feeders of every ilk in the cellphone industry in a 'get-while-the-getting-is-good' frenzy at the consumer's expense. What is even worse is that Apple is so obviously trying to sneak in ever new creative limitations with a straight face - even to the extent of totally ignoring a bewildered debate and questions from the user community for months - 'No comment' is a comment of sorts, right?

As somebody said above - Bluetooth is established technology, and if you sell phones without it you need to make conumers aware of it before they buy - if not you are cheating.
The arrogance of not responding to consumer concerns may not pay off in the long term.

First of all Apple does not respond to these forums. We are all only users helping users here. This is not a limitation Apple has implemented into the iPhone. It is your cell phone carrier that decides whether they will make the Hotspot available on there network and if they will charge an additional fee for it.
Also menu items change between major iOS updates. I am not sure when it was moved, but in iOS 6 there is no longer a General/Networks setting. The Hotspot setting should be at the top of the first Settings screen. It is also found in Settings/Cellular although if it is not at the top of the Settings screen it probably will not be here either. If the personal hotspot is important to you talk to your cell phone carrier for your options.
If you are supposed to have the Hotspot feature on your iPhone you may fix it by resetting or restoring the iPhone. Again, if this doesn't fix it contact your carrier.
Fine, I understand that this is not an Apple-run or -sponsored site. But if you read through this and related discussions you are bound to find that the rollback of functional capabilities through the last 8-9 months of IPhone development has bewildered users. Perhaps the main reason personal hotspot is interesting is that it crops up in discussions about the sudden death of serious Bluetooth capability - unannounced and unacknowledged. It seems that some users have succeeded in getting some measure of the suddenly missing functionality back by fiddling with PH. Others do not find that option and remain unaided by serious information from Apple.
Given that situation, and I think it has lots of parallels in other areas of IPhonery, it would have been natural for Apple to take an interest and say something, if not in this Forum, why not more publicly. The failure to do so is what I criticize, in particular because it seems calculated. And to feel that you, as a consumer, are being taken for a ride makes you feel like a mug, a poor, helpless moron, ready to be conned. That is not a feeling that breeds loyalty and contentment. Several of the contributions to the Forums express that feeling with less restraint than I have done.
To boot, when I have spoken to Apple support personnel they are so completely unaware of the issues that they cannot possibly be meant to assist us punters in understanding what goes on. Now, isn't that precisely what a great con is about?
Settings-General-Cellular- Celular data. Turn it off and back on again. Check beneath Bluetooth in you settings menu. Tada!
Go to Cellular instead. Turn celular data off and on. Then check on your settings menu and PH should have appeared. Worked for me.

In Settings - General, I don't have anything like Cellular etc. So restoring Personal Hotspot is still a mystery. What other ideas do you have?

I am using ios6 and have twice reset my device.
I am also encountering heat and battery discharge on some operations eg camera and accessing public wifi.
Must be a regional thing, I assume "Mobile Data" is your "Cellular" setting.
