Heart rate monitor function on an iPhone?

Has anyone found or created an app that allows an iPhone to serve as a heart rate monitor? If one could merge the functionality of a heart rate monitor, a GPS and a good music player, this could become "the device" for outdoor exercise (e.g., running or cycling). It would allow us to ditch at least one of the devices that are required today.
