It's a form to start a live chat consult with an Apple Business Consultant. Now, what is your real question?
Obviously... I'm talking about how the page hasn't been updated since 2005.
Not "obviously" no; you did not specify what you were asking. I had the same reaction and question about your post, but JK had already responded.
In any case, there's no one from Apple who can respond here and can say anything about it. I don't see a link from the current Apple Store for Business page, though, and the Chat button goes to a server that does not repond, so that probably is an obsolete page that Apple's webmaster has for some reason neglected to remove. You can comment to Apple on the existence of a dead page here, if you wish:

Regards.
It actually was quite obvious; the page looks nothing like any other part of the site and even has 2005 copyright on the bottom. I guess you guys assumedi couldn't read as the answers you gave me were written on the page itself. Regardless, thanks for the link as that's what I was actually looking for prior to posting here as I couldnt remember where it was let alone the word I was looking for (feedback) at the time.
No, it's because your initial question wasn't clear what you were looking for.
But glad you got the answer you wanted.
I assumed nothing, not having any idea what you were referring to, but no matter. For future reference, that link and many other points of contact for Apple can be found by clicking the "Contact Us" link at the bottom of every Apple web page.
Regards.
