Pitch Bend Issues

I've been experiencing pitch bend issues where the note doesn't reset to the original pitch. I first assumed it was my keyboard (Alesis QS 7.1) but now I'm experiencing the same problem with my Roland Ax1. Any insights would be greatly appreciated!
