On the Apple support site, click Apple Care registration (in the AppleCare block), then on the next page insert your serial number.

Jailbreaking is a hack that can brick your device or make it otherwise unusable. Try to restore it to a clean working state by putting it in Recovery Mode: http://support.apple.com/kb/HT1808, and avoid jailbreaking at all costs.
