Basic troubleshooting from the User's Guide is reset, restart, restore. Has any of this been tried?
If none of that resolves, I'd suggest replacing the user or having them evaluated for psychological problems.

If still a problem and assuming you've done the previously mentioned troubleshooting steps, it's a hardware problem.
