Update to IOS 5.1, apps are missing from folders
iPad 2 Wi-Fi + 3G, iOS 5.1
1. Restore from Backup
2. Re-download from: App Store>Purchased

Go to your first home screen and swipe to the right. That will bring up the Spotlight Search. Start typing the names of the apps in the search field at the top. See if the apps show up in the search. If they do show up, they are on the iPad somewhere. Swipe from screen to screen to look for them.
Just because they were removed from the folders does not mean that they are not on the iPad.
I finally figured it out. Not exactly sure why, but some of my apps were automatically restored and were in the correct folders I had created. All of the rest of the apps were still up in icloud; I had to install each one indivdually. Kind of time consuming but it worked.
