Fans running at full speed even after SMC reset.

I have reset the SMC twice on my iMac 24" (early 2009) and my fans still run at full speed, but only after I've been away from my computer for a while. It never happens while I'm using it. Any suggestions? My Apple Care plan ran out a few weeks ago and so I can't call in without paying.

iMac, Mac OS X (10.7.3)

As I said to X423424X, the Hardware Test never initialized using either method. I'm not sure what I'm doing wrong; I tried holding it through the whole restart, releasing and depressing again, and just holding after the computer sounded like it had rebooted (fans beginning to spin, powering on, moving parts moving, etc.). I tried D and cmd D. I suppose since my Apple Care ran out, perhaps AHT is no longer available to me?
