iCloud does it for me. Before iCloud I used to do it utilizing iTunes, but as PeterBreis0807 mentions you need the Pages.app on your iPhone. Take a look at this link:

You need to choose a service to sync your documents across your devices and then install the appropriate software on both your iPhone and Mac. You can use services like iCloud, Dropbox, Skydrive, Google Drive. For all services except iCloud you will need to download and install software for this service. You will need to have Pages installed your Mac (which I assume from this message you have). You will also need to buy a copy of Pages to use on any of your devices (iPhone, iPad, iPod Touch).
