Have you tried turning it on in the Camera app?
Yes. This is actually how I discovered my flash was not working.
Take it to an Apple store fore replacement if you are sure it isn't working. There are a couple of things to check, if you want to. First, there are flashlight apps that will turn it on. Second, you can go to Accessibility settings and turn on the option to signal events with the light.
On my iPhone4, when I carefully press the button to take a picture, then the flash does not work, but if I quickly "peck" it then it does; weird. It's taken me lots of riuned photos to work this one out. Why does Apple do this to us?
Do you have the flast set to Auto or On? If you tap the button quickly the camera may not have enough time to evaluate the lighting conditions, so if it is set to Auto it may not choose the correct flash setting. Note also that the camera takes a picture when you REMOVE your finger from the screen. You can also press either volume button on the side of the phone, and if you hold a volume button it will take multiple pictures in quick succession.
