As to an Apple ID itself, you already created one or you wouldn't be posting here. I presume you mean an iTunes Store account.
There is no iTunes Store in North Korea, and given the sanctions placed on North Korea by the US (and many other countries) there most likely will not be one any time soon, even if NK laws and network infrastructure allowed a Store to be set up. And you cannot use any other country's iTunes Store. Hence there is no need to worry about lack of a credit card since there is no iTunes Store you can use in which to set up an account.
If you are actually in another country, then the link posted above should help, assuming you're in a country that has an iTunes Store, and assuming that you got a real iPhone and not one of the many fakes that appear, particularly in the Asia-Pacific countries.
