Perhaps you, or software you were runing, attempted to cancel your shutdown because it wasn't finished yet, or had open files.
A error occured in this process perhaps.
If it keeps reoccuring I would investigate, using a process of elimination, what program it occurs with and then notify the developer, reinstall it and/or perform #8 Reinstall Just OS X
