If you are 100% sure the headphones are plugged all the way in, it signals an issue with the headphone jack itself. Have you checked to make sure it was clean and clear of debris? If so, it's likely that the jack is damaged and in need of replacement.

Apple helpline just contacted me and recommands a new Nano. I believe the jack might be damaged.
