You could experiment by renaming a copy of your IBA file as .zip, unpacking it, and checking the assets. Delete all copies but one of the audio file. Then edit the XML for the book to point all occurences of the audio file at the same instance.
I haven't tried this myself yet and, obviously, that's a totally unsanctioned way of editing a book. iTunes Producer or some consistency checking tool in the back-end may barf at this. But it should work.
