Randomly, say one in 10 times, when I hang up a phonecall on the Iphone 4s, my flashlight comes on.
I checked the accessibility settings and they are all turned off. I don't want to upgrade to the latest OS
This is frustrating, annoying and of course drains the battery

Try a reset... press the home and sleep/lock buttons until you see the Apple logo, ignoring the slider. Takes about 5-15 secs of button holding and you won't lose any data or settings.
If that doesn't work, you can try to get a warranty exchange at Apple but they will most likely restore it first, which will update it to the most current iOS.
If you have a Flashlight app, try shutting it down or deleting it.
