There might be something on your account. If your card is working for other merchants, best to contact your bank and have them investigate. You can also contact support if you need them.
Is this a debit card that you buy at the store in preset amounts such as $25 or $50? If so, the card will not work in the store. You have to buy one of the prepaid refillable debit cards such as Greendot or American Express Serve cards. These require you to register and have an address on file because the store uses the address as part of it's verification process.
Both of these cards use your bank account to load money to. The Serve card can use your banks debit card to instantly load funds onto and is free of any charges through March of next year.

Yes I would contact your bank. If its a pre paid card it probably has nothing to do with your credit report or credit score. Have you ever though about getting a real credit card? This will make life much easier plus its good for increasing credit score. Hope this helps!
Pre-Paid cards do not work. Period.
They can slide through sometimes, but eventually will give you problems.
