Are you trying to manually apply an update to your iPhone, or are you allowing iTunes to automatically find the latest iOS update available for you?
It's possible that you are applying the wrong firmware for the particular iPhone 4 revision you have -- there are different firmware releases for GSM and CDMA devices.
