Because they are country-specific.
Since you are in the EU the country-specific may not apply. Take it to an Apple Store and ask there.
Is this an iTunes card? I know those only work for holders of an account in the country of purchase. If you have an Italian card you need to have your iTunes Store set for Italy. That requires you be resident in Italy, and enter an Italian credit card billed to an address in Italy.
Even if you have a qualifying Italian account you also need to be in Italy to use it.
The iTunes Store in a country is intended only for use by that country's residents, and only while they are in the country. To use the iTunes Store in a country you need a credit card (or other card type if acceptable in a country) issued in that country, billed to an address in that country, and also be physically present in that country when using the store. You are also restricted to waiting 90 days between country changes.
E.g., "The iTunes Service is available to you only in the United States, its territories, and possessions. You agree not to use or attempt to use the iTunes Service from outside these locations. Apple may use technologies to verify your compliance. - http://www.apple.com/legal/itunes/us/terms.html#SERVICE"

If you look at the circled-hands 1973 UK 50 pence commemorative coin you will see one of the hands is stretched thin and kind of at an angle. I can't track down any other references but I remember at the time being told this was from the design having to be modified because several countries were joining at the same time but Norway stepped out at the last minute. Another suggestion is this is supposed to be Queen Elizabeth's hand.

And even in the EU, the country barriers still apply. The EU has been discussing this matter since 2008 and keeps trying to work toward an EU-wide system for digital purchases, but hasn't gotten there yet.

But how about for gift cards that are not digital purchases between EU countries? I.e. if they purchased the Gift Card (and I am assuming it is an Apple Store Gift card, not iTunes) in an Apple Store in Italy can they then use that Gift Card for an iPod in an Apple Store in France?
I don't know about Apple Store cards. I'm presuming that the OP was asking about an iTunes Card and just mistated, but that may be an erroneous assumption, though from what I can tell with my very sketchy Italian, Apple doesn't offer gift cards for the Apple Store in Italy.

Was actually a follow-up question for someone that I know locally, thanks for the assistance. I wasn't completely clear on the original question either.
