NOTE: Make sure you have done this to Reset the shuffle since noticing this problem

Since you tried both the USB port on Mac and a power adapter, I think you have ruled out (1). You won't be able to rule out (2) unless you have another USB cable or you have another working 4th gen shuffle to test with your current cable. If you can take it to an Apple Store Genius Bar, you can have them try it with their cable to see if that makes a difference. If the shuffle works there, you can by a replacement USB cable, which is this part

If the problem is on the shuffle, and the shuffle is still within one-year of purchase date, it is under warranty. You can see about getting it replaced. Otherwise, if tech support at the Genius Bar can't get it working, I guess you'll need to get a new one.
