First, use this procedure to Reset the shuffle (if you have not already)

If that does not help, as a test, shut down (power off) your computer. Disconnect all USB devices, including USB hub. Connect only standard USB mouse and keyboard (if used normally).
Start up computer. Run iTunes. Connect shuffle to a direct USB port on computer.
If that does not help, please post back (with info about whether it worked before or not).
