With Safari you can't. You could get one of the third-party browsers from the App Store that does have parental controls on it (search for something like 'browser parental') and then hide Safari via Settings > General > Restrictions > Safari 'Off'.

You cannot... It is his Password...He is the only one that can give it to you.
No you can't, only way to reset password is to Restore the iPad with iTunes.
If you paid for the iPad and are paying for the service he uses to access the internet, I'd say you are within your rights as a parent to restore the iPad as new and set it up as you deem appropriate for someone of his age. Point out to him that he will lose everything on it should you decide to do this. Perhaps that will give him the incentive he needs to provide the information.

Diplomacy is key... :-) When I meet with school parents, I reinforce that they should know all of their children's passwords. I mostly work with elementary school kids, so it's easier (problem is the opposite - getting the kids to stop telling them to everyone or saying them aloud as they type, I'm trying to teach them to understand the need for a password), but I know some parents who insist on that all the way through college ("I'm paying for your education, so I get to know your passwords").
My personal opinion is that, if the parents haven't succeeded in inclucating their kids with their values and moral codes by the time the kid is 18, it probably isn't going to happen. 😁
Do not give him any money for the allowance then you will have his password.
