Why does my screen freeze after it goes to sleep?

I have a Mac OS X Lion version 10.7.4

If I don't shut down my computer after each use and it goes into sleep mode, when I return the windows are frozen and when I move the mouse around, it blanks out the screen.

I have been able to figure out that pushing the "help" key on my keyboard, will stop the mouse from blanking out the screen wherever it moves across, however, all the windows remain frozen and I cannot click on anything even to try and shut down my computer. I have to shut it down by using the power button.

Once I restart my computer it is fine, but this problem will reoccur whenever it goes into 'sleep' mode.

Any help here?
