Are the headphones plugged all the way in, meaning the white part of the headset is flush with the body of the Nano?

Since you have already done a reset, have you tried a full restore in iTune? This process will wipe all data from the iPod, reset all settings back to their defaults, and reload a fresh copy of the iPod's software onto the device.
