My iPhone will not display the 4G icon even when I know I'm in a 4G coverage area, so I'm assuming I'm not getting 4G connection. What could be the issue?

Yes, this happens on some networks, but just because it displays 4G, this does not mean that you will get it.
As has been said, an iPhone 4s is not capable of supporting 4G, so you can't get 4G on a 4s. If you want 4G, you need to upgrade to an iPhone 5.
