YouTube Icon has gone from both my Ipad 2 and Iphone 4S
I have looked in restrictions, YouTube isn't shown there, I have checked against age related restrictions and all ages are set as not restricted.
I have done a full reset both via Itunes and pressing the off/switch screen buttons.
Any further suggestions welcome !

if you from the first today screen slide left so you end up in the search screen and you type in
youtube does it find the app?
if so the likely you moved the app to an under folder
if not then likely you installed ios6 beta which no longer include the youtube app
Yes searched for the app no luck, using IOS 5.1.1
Have you tried going to General>Settings>Restriction, and turn it off?
