I would hope not or is the update through the apple web site good enough? I don't want to download 10.8.1 and be charged another $20.

Updates are free.
When you "upgrade" say from Lion v10.7 to v10.8 Mountain Lion, then yes, you must pay for the upgrade.

Maintenance updates (10.8.1...) are free. You have to pay Mountain Lion, but not its updates
So even though app store says $19.99 and I already paid said amount I can install 10.8.1 for free?
Install 10.8.1 in > Software Update. If you purchased Mountain Lion and you can't download Mountain Lion, go to Featured tab, select Account > Show hidden purchases

Software update/app store do not work for me, yet when I click the apple menu it shows 10.8.1

Restart your Mac after the update is installed.
