Diagnosing cause of kernel panic on a Mac Pro

I've got an early 2009 Mac Pro with the following specs:


2.66 GHz Quad-Core Intel Xeon

16 GB 1066 MHz DDR3 ECC

NVIDIA GeForce GT 120 512 MB

OS X 10.8.1


Problem is, at random times, my Mac would just shut down and then restart after I clicked through the "gray your Mac shut down without telling you because it's selfish" warning screen.


I took it to a local computer shop and they couldn't really tell, and suggested I do a clean install of Mountain Lion. I did that, got everything back on from Time Machine (just Documents and Music, no Libraries per the technician's instructions), and then last night it did it again. Restarted while I was working on a web site. No notice.


I wondered if someone could help me diagnose the problem or problems(s), since I can't make ANY sense of the sytem report.

That is really interesting, but the other posters on this thread have Mac Pro 65 lb tower computers, and most of them have multiple NVIDIA graphics cards installed. You do have a similar kernel panic:

If you want Apple to help you, they will ask you to replicate the problem with all these add-ons Un-Installed.

It is quite likely that this problem showed itself with the Mac OS Update.


But that does not implicate the Mac OS X software.


Sometimes pre-existing problems are made worse by later versions using the Hardware in a different way.


Much more often, the versions of add-ons are not appropriate for the updated software, and this ends up being a problem caused by third-party add-ons, not Apple software at all.


If you take it to the genius Bar, they run their diagnostics. If nothing is found, they will start you doing a complete re-Install, but without any third-party add-ons. If you can get panics to happen after that, THEN they will pay attention and help you.


The one other thing you can do on your own is to run Apple Hardware Test. Unfortunately, It is know for false negatives.


Your list of panics is impressive, but each panic needs to be analyzed for individual failures. Kernel panics are never "all the same".

I have probably the same problem on an early 2009 (Nehalem) Quad Core Mac Pro 2.66 with three NVIDIA GeFOrceGT 120 cards driving 3 dual link DVI 30 inch cinema displays. The graphics interface is admittedly a hardware hack, but that is what Apple offered as a replacement for a 2008 model that died at 12 months of age. Despite the clumsy graphics hardware, I have not had any problems or Kernel Panics in the prior 3 years with this system before upgrading to ML X.8.2 (directly from X.6 SL). Hardware as been constant since 2009. My system is now hard crashing (not even a gray screen) about once every 36 hours for last 8 days. This is an unacceptable situation and I'm considering reversion back to X.6.


I'm no expert an interpreting the panic reports, but the first part of the report is similar to those above with iokit or NVIDIA driver interface a likely culprit. All have been forwarded through crash reporter to Apple. Please advise if anyone has any additional ideas. Thanks,

I think i have this same problem and am dropping my report here after I took the machine into apple and they said it was fine. I updated today to Mountion Lion from Lion thinking the issue would resolve but it is crashing more.


It hates restarts, and installs make it crash. Its too unstable to use at work even for simple tasks now let alone editing.

That is really interesting, but the other posters on this thread have Mac Pro 65 lb tower computers, and most of them have multiple NVIDIA graphics cards installed.


Their Mac Pros have error-correcting RAM memory, your MacBook Pro does not.


Yours is an ordinary page fault with no non-Apple kernel extensions loaded.


It panic-ed in routines called by mds (Spotlight). That could indicate disk problems, but does not rule out memory problems.


Do you have any other panic reports available?

I ran this yesterday for 2 hours and got a clean bill of health for the RAM.

MemTest for Mac OS X Tests your RAM


The drive was replaced (had a couple flaws) clean install - updated - Time machined - then upgraded to mountian Lion.


I cleaned the library and registry for a couple old apps removed Littlesnitch and Mackeeper.

Similar experience here...new drives, I just removed the RAID card completely, clean install of ML and still multiple Kernel Panics. The RAID card had a few issues, but does not seem to be the main problem...still crashing with RAID card out and brand new normal drives in. Crashed 4 times yesterday without warning. Each time, I changed one piece of hardware...unplugged one monitor, then removed the 3rd monitor, then the 3rd graphics card GT120 graphics cards. Technicians at Apple Store tested all hardware too and could not find the source (check backend records at twentyninthstreet store). Harware is likely irrelevant because all hardware passes all tests each time. This is a ML software or driver issue.


Very distrubing to have to compromise this expensive system so much in search of early 1990's level stability. Cudos to the local Apple Store (Boulder Colorado) tried to help and even provided services for free despite being a few months out of warranty, but no improvement--hard to complain about them despite no resolution. I forked out some money for the mini display to dual link DVI adapter (painful) and am down from 3 to 2 graphics GT120 cards:

(which hardware check fine) and forced compromise of using minidisplay port on just one card is unfortunate to loose the video RAM and have to buy an expensive adapter to preserve the output to my perfectly functioning 30" Cinema Displays--and going down from 3 to 2 display with this strategy--all because of driver support on all stock Apple equipment. The expensive triple 30" display strategy is otherwise functioning perfectly. I will keep the thread posted if it still crashes after sacrificing monitor(s). This is definitely suboptimal--hope the engineers can make something out of these crash reports. Here is my latest kernel panic...hope it means something to someone who can engineer a patch. This is similar to multiple prior reports despite hardware reconfigurations described above (iokit and drivers are the culprits):

Starting afresh with new account on the machine I am working through possible software issues for kernel conflicts and rebooting over and over to replicate issue,


I removed all Application support for previously deleted apps such as Littlesnitch and Mac keeper.

After going through this discussion I deleted Google Chrome - Just in case.

I did find one confirmed conflict with Netgear Genie - And deleted this program after the system seemed stable but this program created consistant crashes / Kernel panics.
